Weight-Loss Injections and Skin Health: What Does Nutrition Have to Do With It?

GLP-1–based medications have transformed the treatment of obesity. Semaglutide, tirzepatide, and other drugs in this therapeutic class can lead to substantial weight loss and important metabolic benefits. However, as their use has increased, questions have also emerged regarding possible changes in the skin, face, and hair.

Skin laxity, loss of facial volume, a more aged appearance, and hair loss are among the concerns reported during treatments that result in significant weight reduction.

However, one point is essential: these changes do not necessarily mean that GLP-1 medications directly “age” the skin.

Much of this phenomenon appears to be related to the magnitude and, particularly, the speed of weight loss, with reductions in subcutaneous fat, changes in body composition, and, in some patients, inadequate intake of protein, vitamins, and minerals.




What Happens to the Face When We Lose a Significant Amount of Weight?

The skin does not exist in isolation. Facial appearance depends on the interaction between the skin, adipose tissue, muscles, ligaments, and bone structure.

During significant weight loss, there is a reduction in the fat compartments that help support facial tissues. Studies describe volume loss, particularly in certain areas of the face, accompanied by increased skin laxity and greater prominence of pre-existing folds and wrinkles.

This is where the popular expression “Ozempic face” comes from.

However, the term may be misleading. Facial changes appear to be primarily associated with rapid weight loss and loss of facial fat, a phenomenon that can also occur after other forms of substantial weight reduction.

In other words, these aesthetic changes should not automatically be interpreted as direct pharmacological toxicity of semaglutide or other GLP-1–based medications.


Fat Is Also Part of the Skin’s Support System


When we think about skin health, collagen and elastin usually come to mind. But another component is also important: subcutaneous fat.

The adipose tissue located beneath and around the skin contributes to its support and influences facial volume, contours, and mechanical properties. When a significant amount of fat is lost, this supporting structure is reduced.

Scientific literature suggests that loss of adipose tissue may be associated with reduced skin firmness, changes in collagen fibers, and increased skin laxity.

For this reason, obesity treatment should not be evaluated solely by the number on the scale.

Healthy weight loss means reducing excess body fat while preserving, as much as possible, muscle mass, physical function, and nutritional status.


Where Does Nutrition Fit In?


This is one of the most important aspects of treatment.

GLP-1–based medications significantly reduce appetite. This is part of their therapeutic mechanism, but it may also lead some people to eat considerably less — without necessarily eating better.

Reduced food intake can make it more difficult to meet nutritional requirements, particularly when patients follow highly restrictive diets in addition to medication.

Therefore, reducing calories should not mean reducing the nutritional quality of the diet.


Protein: A Priority During Weight Loss


Adequate protein intake deserves particular attention.

Protein provides amino acids required by multiple tissues and is also essential for preserving muscle mass during weight loss.

This is especially important because weight loss induced by GLP-1–based therapies does not occur exclusively through fat loss. Some reduction in lean mass may also occur.

For this reason, nutritional strategies during obesity treatment should include adequate protein intake together with resistance exercise whenever appropriate.

In practical terms, this means distributing appropriate sources of protein throughout the day according to each individual's needs.

Animal protein sources may include:

  • fish;

  • eggs;

  • poultry and meat;

  • milk, yogurt, and other dairy products.

Plant-based sources include:

  • beans;

  • lentils;

  • chickpeas;

  • soy;

  • tofu;

  • peas;

  • nuts and seeds.

The ideal amount should be individualized according to factors such as age, body composition, physical activity, clinical conditions, and the degree of caloric restriction.


Vitamins and Minerals Also Matter


Healthy skin and hair depend on adequate nutritional status.

During periods of substantial food restriction, several micronutrients may deserve attention, including iron, zinc, copper, vitamin C, vitamin B12, vitamin D, and other vitamins and minerals, depending on the patient's diet, symptoms, clinical history, and laboratory findings.

Vitamin C, for example, plays an important role in collagen synthesis.

Zinc is involved in cell renewal, immune function, and wound healing.

Iron is essential for oxygen transport and may be particularly relevant when evaluating hair loss.

Copper participates in processes related to connective tissue structure, while vitamin B12 is important for normal cellular and neurological function.

This does not mean that every person using a GLP-1 medication needs supplements.

Supplementation should be individualized and based on clinical assessment, dietary intake, and laboratory testing when appropriate.

Taking multiple supplements without identifying an actual nutritional need is not necessarily beneficial and, in some cases, may even be harmful.


GLP-1 and Hair Loss: Is There a Connection?


Hair loss has also become an area of scientific investigation.

Observational studies and pharmacovigilance analyses have reported associations between GLP-1 receptor agonists and alopecia or hair loss. However, determining whether this represents a direct drug effect is difficult.

Rapid weight loss itself can trigger telogen effluvium, a form of diffuse hair shedding that may occur after significant physiological stress.

Marked caloric restriction and inadequate intake of protein, iron, zinc, and other nutrients may also contribute.

Therefore, when significant hair loss develops during treatment, the clinical and nutritional context should be evaluated rather than simply attributing the problem to the “weight-loss injection.”


Can GLP-1 Medications Also Have Beneficial Effects on the Skin?


Interestingly, the relationship between GLP-1 and skin health is more complex than the expression “Ozempic face” might suggest.

Recent research has investigated potential anti-inflammatory, antioxidant, and immunomodulatory effects of GLP-1 receptor agonists.

Preliminary studies have explored possible benefits in inflammatory skin conditions and wound healing. However, much of this evidence remains early, and GLP-1 receptor agonists should not currently be considered established dermatological treatments for these conditions.

This creates an interesting apparent paradox.

While rapid fat loss may reduce facial volume and tissue support, the metabolic and anti-inflammatory effects associated with GLP-1 therapies may potentially have favorable effects on other aspects of skin biology.

More research is needed to fully understand these mechanisms.


How Can You Protect Your Skin During GLP-1 Treatment?


Several practical strategies may help support skin, muscle, and overall nutritional health during treatment:

  1. Avoid excessively rapid weight loss whenever clinically possible.

  2. Ensure adequate protein intake throughout the day.

  3. Maintain a varied diet based primarily on minimally processed foods.

  4. Ensure adequate intake of vitamin C and other essential micronutrients.

  5. Assess iron, vitamin B12, zinc, copper, and other nutrients when clinically indicated.

  6. Maintain adequate hydration.

  7. Perform resistance training regularly, when medically appropriate.

  8. Avoid extreme caloric restriction.

  9. Monitor body composition, not just body weight or BMI.

  10. Maintain multidisciplinary follow-up involving a physician, dietitian or nutrition professional, and, when necessary, a dermatologist.


The Goal Is Not Simply to Lose Weight


GLP-1–based therapies represent a major advance in obesity treatment. But the more effective pharmacological treatment becomes, the more important it is to evaluate how that weight is being lost.

Substantial weight reduction can affect facial fat, tissue support, muscle mass, dietary intake, and nutritional status.

For this reason, nutritional care should not be viewed merely as an additional strategy to increase weight loss.

Its role is also to help preserve muscle mass, prevent nutritional inadequacies, and support skin and hair health throughout treatment.

Ultimately, successful obesity treatment should not be defined simply by achieving the lowest possible number on the scale.

The goal is to lose weight while preserving health, strength, physical function, nutritional status, and tissue quality.
